LOGISTICS:  Applicants will be notified of
selection by June 27, 2007.   Students/parents
are responsible for transportation to and from
the program.  Transportation for field trips is
provided.  Food and snacks are included.
This form indicates student interest only.  Upon approval, this entire form must be printed, signed by the
parent/guardian and accompany the student on the first day of camp to complete registration.  Insure that
you get an email confirming that this form has been received.  ELIGIBILITY:  Participants must be students
between the ages of 12-18.  Applications must be submitted by May 23, 2008.  Include the $25.00 fee with
application.
